How can I force a sub-total of all expenses higher up in the report at a particular spot of my choosing in the income and expense report?

Adding Payroll Accounts
We have not previously used payroll accounts, but now need to add them. Is there a sample chart of accounts somewhere so I can see the way a default or basic chart of accounts including payroll is set up?

Overpaid pledges
When a annual pledger overpays his pledge before the end of the year, and we print a statement for them, it shows a positive balance on the "Pledge Balance Outstanding" line.
