Attendance - Attendance Registration Report

I tried to run the "Attendance Registration Form", "weekly column" format report but couldn't get it to work. I got a list of names but not the weekly date columns that I usually get. I've used this in the past and wonder if there is a problem with the new releases?

How did it look when you ran it to display on the screen ?
