What is the variable to add last attendance date to a custom report?

I have created a custom report that I need to add one more thing to. I noticed in a standard directory there is a question to add an aterisk to children and a footnote that explains that is for children or for members, etc. How is this done and is it possible on a custom report?

I would love to share custom reports. I have a great one for the church directory that lists the last names, then the parents names, addresses, phone numbers and kids on a separate line and also displays which parish they are in along with deacons names.
