Child Consent Forms

Our church policy requires that we have an annual consent form signed by parents. On the form would be the usual address, phone number, emergency contact #'s and if we are allowed to use photos on social media. Wondering if there is such a form that powerchurch could generate with each child's name and information to simply be signed by the parents yearly (rather than them filling out all their information on a form every year and us having to check that it's all correct in Powerchurch!)??? And information that would be incorrect on the form, the parents can correct it on the form and that way we know exactly what information needs to be edited in Powerchurch.

Well, you could use the Family Grouped Report under Personal Profile Reports to get the entire family printed out and use that as the data check, however, depending how you have the Personal Profile Status Codes setup, if you select member, you would also get the report on members without children.

If you're OK with that, all you would need to do is pull out the Report for those families with children, and attach a cover sheet with the consent text for them to sign along with checkboxes like: information correct, information updated on form, etc.
