Is there a report that list the names, address and phone number for the Sunday School Teachers?

There is a report called Address & Phone Directories in Activities & Skills. Run that and choose to only include Roles of "Teacher" or whatever you call your teachers in the program.

I had already checked that one. I need it all on one page.
Is there anything else?

Well, you can save it as a RTF file, then edit it in Word to eliminate the white spaces between groups. Other than that advice, there is no report that will do exactly as you ask.

The alternative would be to create a custom report, which would probably be more labor intensive than just editing the Word or Excel export of the existing report.
