Visiting Log for the Sick

We are trying to come up with log/report for when the pastor or a deacon visits a sick/elderly member of the church. I know that there is a visitations under membership, but what do I do when we want to schedule followup visits. Any input on how to enter and keep track of visits would be greatly appreciated.
