Birthday Report - Preferred Name

The birthday report does not use the preferred name in its report. I have an instance
where the person's middle name is the preferred name and preferred is checked as the
option for reports, but the birthday report uses the first name rather than the preferred.
Either this should default to preferred if selected in the profile, or the report should
allow this selection in the options window when the report is initiated.
