Additional withholding

Our pastor has elected to have taxes withheld from her check. On her W-2, she asked me to withhold as M-1 plus an additional $125 to cover her self-employment tax. I set it up this way in PowerChurch and it worked perfectly for the first month or two, then it suddenly quit working properly. It now only withholds the $125 and I have to change it each payroll. Did something change in program, or have I accidentally changed something to make it quit working?

Make sure that the Pay Item for Federal Taxes as shown on the Pastor's payroll record under Maintain List of Employees still has the proper federal tax table listed.
