I was working offline on this, so I thought I would post it here.
This is my best guess at how to set up a Canadian Payroll Tax:
Create the Liability account (01-2217-000) or whatever account number you created
Payroll / Setup / Maintain Tax Types
ADD
Local Tax
Description: Provincial Tax
Locality Name: Ontario
ID: 123-321-123
Payroll / Setup / Maintain Tax Tables
ADD
[dropdown box] Generic Flat Tax? Generic Percentage Table? (fill in the necessary information accordingly)
Description:Provincial Tax Table
Tax Type: [dropdown box] Provincial Tax
Payroll / Setup / Maintain Item Descriptions
ADD
Tax Deduction
Item Number: 17 (or whatever assignment you choose)
Description: Provincial Tax Item
Liability Acct: 01-2217-000
Bank Acct: 01-1110-000
Tax Type: [dropdown] Provincial Tax (you created this earlier)
AP Vendor: Bank
Payroll / Setup / Maintain List of Employees
Pay Items
(lower left) ADD
[dropdown] Tax Deductions

Amount: (enter amount if flat tax amount, or choose Provincial Tax Table which you created)
