Federal tax reporting issue

Good morning,
An employee changed her w4 mid quarter...as a result we deducted $39.00 too much for her federal tax on one check. How do I repay this to her and how do I show the correction on my quarterly 941 report? Must I complete a 941x?

FWIW .. I wouldn't do anything. That amount has already been deposited and reported. If it were not, you could do an adjustment, but that money has probably left the church. She'll get it back when she does her 2014 taxes.
