Getting church name to show up on contribution statement

Hi,
We are currently running version 11.5 Is there a way to get the church name to show up on the contribution statements?

If I remember right, it should appear in the return address at the top of the form.

The only thing that shows up on the page is just a blank sheet with the contributions listed. Normally we put it on letterhead so it is not a big deal, but we would like it to show up this year...just not sure how to make that happen...

I figured it out...you go into contribution set up and there is a place to check if you want the name to appear or not...thanks for the help!

That explains it ... checking that
Print on Letterhead option does eliminate the name from the return address block.

I have a different question and hope you can help me.
I would like the number zero to show up on Contribution statements instead of 9999.99 when the person has not pledged. Can you help??
