Custom Reports for Children's Ministry & Youth Ministry

I need to create a custom report that will allow our Children's dept. to continue to generate "attendance" and sign in sheets for their services. The requirements are kind of quirky and I'm at a loss right now to produce the reports they are looking for based on the way we have PC setup currently.

Children's Ministry needs: (this is what they are currently working with through Access)

Last name, First name, Allergies, Other Relevant Info, Date of service, Card Number, Parent Location.

The above are the fields that they have on their sign in sheets for each service. All fields are already populated with the relevant info except Card number and Parent location.

Attendance reports don't get as detailed as they are requesting so I am completely lost.

I truly believe that I will have to go back and almost completely set PC up again to meet these needs. This probably won't even make sense to anyone else since you guys don't know how we have PC set up.

I'm just at a loss and grasping for straws right now.

Any help will be greatly appreciated.
