Attendance Register Form which provides past attendance

I am looking for a way to reduce the amount of paper I give to classes each week as well as simplify the attendance process. I want to know if anyone has a report that includes the name and contact info of a member as well as a 4-6 week quick summary of attendance (such as a string - PPAAAP indicating attendance in the last six weeks). With this type of form, the class secretary would have all the information needed to know if a contact is needed and how to contact the member.
