Attendance Register Form

I have several activity numbers I would like to list on one "Attendance Register Form" style of report (single page). In the "Reports - Attendance" module, you have a default report (weekly column format) that prints each activity number on a separate page. For example, each of my Sunday School classes are broken into teachers, members, visitors, outreach leaders, etc., each with an independent activity number. Your default report places each of these "activities" on a separate page and I would like to include them in one attendance form. I have used the custom report section to try and group these multiple activities together; however, I cannot locate the field/variable/expression to select future dates for which I would like to keep attendance. The default report allows me to select a range of dates to print on this report and it is critical for maintaining correct attendance. Could you tell me where that field/variable/expression is located so that I can include it in my custom report?

Version 9 now allows you to keep these 'teachers, members, etc' under one activity number, using the activity role, so there is no longer a need to use separate actitvities.

If you're in a version prior to 9, in the custom report, you would just have to type in the dates you want to include. You wouldn't be able to pull these dates from a field. Just add in a text box and type the date there. These dates would have to be changed each time you ran the report.
