W-3 form

I am having problems with the 2011 W-3 form. The program is putting an extra dash in the EIN and removing one of the numbers. Also, it is not filling in the contact, e-mail, phone number, etc. I tried manually inputting these numbers but get a message "control is read only". I remember being able to modify the W-3 in the past. Now i will have to white out/write in certain fields for the W-3.

If you are using Version 10.x, this was fixed in the MR dated 01/09/12.
